The Academic Records Office manages and issues academic records and university documents at the Iuliu Hațieganu University of Medicine and Pharmacy in Cluj-Napoca.

Documents required for the issuance of academic records and university documents such as diploma supplements/transcripts:
- Valid identity card or passport (original)
- For academic documents with a “photo space”: 2 recent color photos, 3×4 cm, taken at a professional photo studio, on porous photo paper (only if they have not been submitted to the faculty);
Please note: Inappropriate photos will not be accepted on the diploma.
Issuance of academic documents and university documents such as diploma supplements/transcripts:
Academic documents and university documents such as diploma supplements/transcripts are issued to the holder within a maximum of 12 months after passing the final exams, or within a maximum of 2 months after being awarded a doctoral degree.
If the holder is unable to attend, these may be issued to his/her representative, on the basis of a power of attorney certified by a notary public/attorney, which expressly states that the representative may collect academic records/university documents.
If the power of attorney was drawn up in another country, the original must be submitted, accompanied by a certified translation. The power of attorney must be apostilled or superlegalized by the competent authorities of the country where it was drawn up, except for those drawn up in countries that have concluded conventions, treaties, or agreements with Romania on legal assistance in civil matters, which provide for exemption from any legalization. Powers of attorney drawn up at Romanian embassies or consulates in various countries are accepted. Powers of attorney signed by the ambassador/consul do not need to be translated or apostilled/superlegalized.
Issuance of duplicates of educational documents:
In the event of loss, complete destruction, partial damage, or lamination of an educational document, a duplicate may be issued.
Documents required for issuing a duplicate of an educational document:
- Application for a duplicate, completed by the holder of the document or their representative
- Written statement by the holder of the document, containing all the necessary identification details and the circumstances in which the document was lost, destroyed, or damaged
- Certified copy of the birth certificate (the original must also be presented)
- Two photographs – recently taken on photographic paper, in color, 3 x 4 cm in size – of the holder of the document;
- Proof of publication in the Official Gazette of Romania, Part III, in the event of loss or destruction of the educational document;
- Proof of payment of the fee for issuing the duplicate
- Duplicate diploma – 300 lei
- Transcript – 300 lei
- Diploma supplement – 300 lei (the transcript has been replaced since 2006 by the diploma supplement).
- The damaged or laminated document, in original form;
- Notarized power of attorney/legal representation (if the diploma is collected by a representative)
IMPORTANT!
- The request for a duplicate will not be sent to the “Iuliu Hațieganu” University of Medicine and Pharmacy in Cluj-Napoca without all the documents mentioned above.
- Documents that are not issued in Romania require an Apostille from The Hague, if the country in question is a member of the Hague Convention. If the country in question is not a member of the Hague Convention, the documents will be legalized (at the Embassy or Consulate).
- All documents are issued in Romanian.
If a duplicate of an academic document has been lost, damaged, destroyed, or laminated, the holder will be issued another duplicate upon request.
The deadline for issuing a duplicate of an academic document is a maximum of 30 calendar days from the date of approval of the request.
Instructions for issuing certificates:
- STUDY CONFIRMATION CERTIFICATE, REQUIRED BY THE MINISTRY OF HEALTH/DSP, FOR THE CERTIFICATE OF CONFORMITY
- CERTIFICATE OF AUTHENTICITY REQUIRED BY THE MINISTRY OF EDUCATION (only required for graduates who completed their studies before 2017)
- Fill in the standard application form (Compliance Application)
- Attach simple copies of your study documents (bachelor’s and master’s/license degree, diploma supplement/transcript)
- Attach proof of payment of the certificate issuance fee
The documents must be submitted to the university registry, 8 Victor Babeș Street, ground floor, or sent by email to the following address: registratura@umfcluj.ro.
The certificate issuance fee is 50 lei and must be paid in lei.
